Zu Hintergrund, Fragestellung und Bedeutung der Publikation:

Lange Zeit galt der operative Erhalt des vorderen Kreuzbandes (VKB) auf Grund ungünstiger Langzeitergebnisse und einer erhöhten Rezidivinstabilitätsrate als obsolet. Vor dem Hintergrund innovativer Operationsmethoden hatte der VKB-Erhalt in den vergangenen Jahren eine Renaissance erfahren; Kohortenstudien konnten vielversprechende klinische Ergebnisse belegen, jedoch fehlte es an kontrollierten Studien.

In der vorliegenden Arbeit konnte in einerm prospektiv-randomisierten Design an 85 Patienten gezeigt werden, dass die klinischen Ergebnisse nach VKB-Erhalt mittels Dynamischer Intraligamentärer Stabilisierung (DIS) im Falle einer akuten Verletzung des VKB vergleichbar mit den Ergebnissen nach VKB-Rekonstruktion mittels Sehnenersatzplastik sind.

In der vorliegenden Arbeit wurde der operative Erhalt des VKB mit einer modernen Operationstechnik (DIS) erstmals in einer prospektiv-randomisierten Studie untersucht. Die Ergebnisse belegen, dass die DIS bei akuter Verletzung des VKB eine Alternative zur Rekonstruktion darstellt.

Background and fundamental question of the publication:

Historically, repair of the anterior cruciate ligament (ACL) was associated with poor long-term results and high rates of recurrent instability. Recent advances in surgical technique have broad ACL repair back into focus. Newly developed techniques, i.e. dynamic intraligamentary stabilization (DIS) showed promising results in cohort studies. However, the level-of-evidence is still too low to support the use of such new techniques.

The present study provides 2-year results of a cohort of 85 patients who underwent either ACL repair with DIS or reconstruction in a randomized manner. Functional results after ACL repair were comparable to those of ACL reconstruction. Young age and a high activity level are the main risk factors for recurrent instability after both ACL repair and reconstruction.

Level-1 evidence in a study with a large number of patients and with a sufficient follow-up period is provided for the first time. Based on the results of the present study ACL repair with DIS can be seen as an additional option to treat patients with acute ACL tears.
